If during For the Advancement of Learning quest you didn’t convince Keira to stay away from Radovid she will attempt to parley with him and be executed.

Geralt learns about this when he suggests to Triss that Keira could be one of the lodge members. Triss tells you that her body was impaled. You have the option of going to retrieve her body and giving her a proper burial.

Keira’s body is located in the Hierarch Square. You can either retrieve it peacefully by using the Axii sign or by fighting off the guards. Afterwards Triss will take the body to give Keira a proper burial.
