The Spear of Destiny forms part of The Blood Curse quest. When you meet Zoltan and Saskia in the tunnels they’ll tell you that Iorveth had the spearhead but lost it playing dice poke to Skalen.

You can find Skalen by the Gates of Vergen. Challenge him to dice pocker and play him until you win. When you do he’ll give you the Spearhead. If you beat him two more times he’ll also give you the Dun Banner Cloak but this isn’t an essential item.
